GAL Travel Deposit Form

Initial Travel Deposits are due by June 30, 2008
(and sooner is better!!!)

This form is only for travel to San Francisco, CA for the 2009 Chinese New Year Parade. Deposit due at this time is $100 per person. Please note that this deposit is NON-REFUNDABLE. See the Event Information page for full trip details and pricing information.

IMPORTANT NOTES:

If you supply new credit card information, Tom must have the exact billing address for the credit card.
If you supply new credit card information, Tom must have the three-digit number that's called the Card Security Code or Card Verification Value (CVV). This three-digit number is located on the back of your credit card.
Please make sure names of all travelers match the identification that they'll use when checking in at the airport. Your airline ticket needs to have the right name or you'll have trouble at the airport!!!
If you only need the land portion of the trip, please note that in the comments section and contact Tom for pricing. This is useful if you're making your own air reservations since you're traveling from a city other than Portland and just want hotel and bus reservations.

You have three options available in the Amount to Charge section:

If you choose "First Deposit Only", Tom will charge your card for only the first deposit now, then charge your card again later for the second deposit and balance payment as described on the Event Information page.
If you choose "First and Second Deposit", Tom will charge your card for the first two deposits now, then charge it again later for the final balance as described on the Event Information page.
If you choose "Payment in Full", Tom will charge your card now for the full amount of your trip.

There are three ways to use the Payment Options section::

If you traveled to Florida with GAL in January and used a credit card to pay for your trip, you can use the "Credit Card on File" option and Tom will charge the deposit to the credit card you used for the Florida trip.
You can choose a credit card type and include credit card information. Your credit card will be charged for the appropriate deposit amount.
Include everything on the form except credit card information. In this case, you are responsible for calling Tom with your credit card info or ensuring that you get him a check or cash. Choose "Contact Tom Directly" in the payment options below if you choose this method.

Just so you know: This form is not encrypted when it gets sent across the web. Your credit card information is placed in an email message that is sent ONLY to Tom and me (Steve) to keep track of things. This is pretty low risk (it's exactly the same as if you sent an email message to Tom with your credit card info), but we want you to have the option of just calling him up if you're uncomfortable.

Tom will send you an email acknowledgment so you know your form has been received.
