Long Beach Loyalty Days Travel

This form is only for travel to Long Beach, WA for the 2010 Loyalty Days Parade. Cost of travel is $20 per person, payable as noted below (the band is partially subsidizing the cost of the bus). See the Event Information page for full trip details.

Credit cards cannot be used to pay for this trip; you need to pay via check or cash only. Checks should be made payable to Get a Life Marching Band (note: do NOT make checks payable to Bob). Please deliver your payment to Bob or mail your payment to him at:
